Institutional form used to verify completion of an educational program and to recommend the educator for licensure (teacher, school counselor, school psychologist, and administrator). | | Instructions
The Program Completion Report (Form C-2) is a recommendation form completed by your college or university where you completed a teacher, administrative, school psychologist, or school counselor program. Adding endorsement and levels can also be verified with the C-2. This form recommends for the levels and endorsement area(s) that qualify you for licensure. Please note on the Teacher Education Program area, Oregon has four levels-Early Childhood (age 3-grade 4), Elementary (self -contained grades 3-8), Middle Level (grades 5-9 in a middle school) and High School (grades 7-12 in a high school). The form is signed by the Director of Teacher Education and sealed in an institutional envelope. The form will not be accepted if received unsealed.
